The Bears launched their fifth annual 12 Days of Bears Giveaways.

The team will give away a new prize every day for the next 12 days. Details of this year's daily prizes will be announced each day on the entry page. Prizes from previous years included Bears knit hats, bobbleheads, gift cards and autographed merchandise.

Fans can enter by filling out the form available by clicking here. Interested fans can enter the sweepstakes every day before 11:59 p.m. to be eligible for that day's prize.

Fans can double their chances to win by entering via the Chicago Bears Official App. Click/tap here to download the app or to go right to the entry page if you already have it on your mobile device. 

Once the app is opened, tap on "more" and then tap on "12 Days of Bears Giveaways" to fill out the form to enter the sweepstakes. The Bears will send daily push notifications through the app to serve as reminders for users to enter to win each day's prize. 

Fans can also follow @BearsLocal on Twitter to find out each day's prize.

Sweepstakes runs until Thursday, Dec. 24 at 11:59 pm Central Time.
